Health ServicesOur health services include health screenings such as blood pressure
checks, cholesterol levels, blood sugar levels and foot exams by nurses,
podiatrists, and other professionals. Call for more information. TransportationWe provide transportation for seniors to participate in services at our centers and for special appointments. Escort service is provided by
having a companion to accompany seniors on trips such as to their doctor,
shopping and banking. Community ServicesThis includes meaningful classes, sewing, knitting, and exercise as
well as other creative, social, educational, physical, psychological and
economical activities. Consumer ServicesWe provide assistance with completing forms such as renter's rebate and
tax preparation forms. Special Events & Recreational TripsA full range of special programs
and recreational trips are provided
throughout the year to meet the needs of seniors. these activities are
designed to keep seniors active, healthy and involved. Seniors are
challenged mentally and physically to enable them to continue to enjoy
life in their later years. Outreach ServicesTelephone Reassurance Visiting Case ManagementThe case management program provides comprehensive social services to the at risk elderly and disabled population in the southwestern area of Los Angeles. The program seeks to assist the elderly population in maintaining independent living and to coordinate services which enable clients to remain in their community environment. Trained case workers assist with filing applications for financial and social services. Information and Referral Adult Day CareOur adult day care center is licensed to care for seniors who are diagnosed with Alzheimer's Disease or Dementia, as well as Stroke survivors and the frail elderly. The Alzheimer's and Dementia Social Day Care Program and Resource Center is located at: 4703 Crenshaw Boulevard We provide direct supervision and a care plan to maintain current level of functioning and delay institutionalization. Trained staff assist with activities of daily living. We arrange and provide organized social, cultural, and recreational activities. Services provided:
Caregivers are encouraged to become involved in bi-weekly support groups, advisory committee and care planning for their loved ones. Program costs All fees are based on a sliding fee scale. A self-declaration of income is required. The goals of the program are:

& the Frank N. Mellinger Alzheimer's Fund. Caregiver SupportThose who have the responsibility of caring for a senior are welcomed
to attend our support, educational, and
discussion groups. We provide referral to community
organizations and have a resource library with helpful books and
