The program strives to find a situation that maximizes a client's independence and autonomy, including helping an individual remain at home. This includes assessing what level of care an individual might need by evaluating a client's:

| Maintaining or selling real property | |

This is fee-for-service program and you can call to obtain the current fee schedule.
Expert and knowledgeable staff can:
| Assess client needs, | |
| Help create Durable Power of Attorney for Health Care (DPAHC) and/or act as health care agents, | |
| Provide case management services, | |
| Provide money management services, | |
| Counsel families on short- or long-term care planning, | |
| Be appointed as conservator of the person and/or the estate, | |
| Be nominated as trustee of family trusts, | |
| Be nominated as administrator of estate. |
