The JFS / Linkages Program is located at Valley Storefront Multiservice Center for Seniors and other Multipurpose Senior Centers throughout the City of Los Angeles.

Linkages is for people who need help managing their lives because of an accident, illness or other disability. The goal of the program is to prevent or delay the premature or inappropriate institutionalization of at risk, frail elderly and functionally impaired adults 18 years of age and older.

Linkages is designed to be a "gap filler" by serving individuals who are not eligible for other services. the program provides case management as well as information and assistance regarding appropriate community resources. Up arrow to top of page


Eligible adults are:

bullet18 years and older
bulletLiving in the City of Los Angeles
bulletLiving with health and/or environmental problems placing them "at risk" of premature or unnecessary institutionalization.

Linkages clients may or may not be eligible for Medi-Cal.

The Linkages Program is funded by the City of Los Angeles Department of Aging, and is accessible to persons with disabilities as required by the Americans with Disabilities Act. Up arrow to top of page

Linkages Services

The program provides essential links that help promote independence and prevent institutionalization. Linkages coordinates, manages and follows up on services such as:

bulletSupport/relief programs for families
bulletLegal advocacy/assistance
bulletIn-home attendant care
bulletMedical referrals
bulletMoney management
bulletNutritional services

bulletReferral to available resources.
bulletIn-home or hospital assessment by professional social workers.
bulletFinancial assistance for service needs where applicable and available.
