Bullet Case Management

The Case Management program services frail, homebound seniors, sixty years of age and older. The Theresa Lindsay Center is dedicated to serving the frail elderly so they may continue to live independently in their home. The Center's goal is to prevent premature placement in any type of institution.

The case management program is based on a comprehensive assessment of the individual needs, usually done in the privacy of the senior's home. A plan is then developed to help link the senior with the appropriate services and resources. Monitoring and follow-up is provided to assure the needs are being met.
